主 旨︰公告本校114學年度第2學期休學申請截至115年06月18日(星期四)止，希知照。

說 明︰

一、        依據學則第43條規定辦理。

二、        114學年度第2學期欲申請休學之學生於115年06月18日(星期四)前，至學生資訊系統D.1.06a.休、退學申請進行線上申請後，列印紙本申請單並完成申請單中導師、系所主管、院長及各單位核章流程，逾時不予受理。

Description:

1.     This announcement is made in accordance with Article 43 of the Academic Regulations.

2.     Students who wish to apply for a leave of absence for the second semester of the 114 academic year must complete the online application via the Student Information System D.1.06a.Suspension and Withdrawal Application) by June 18, 2026 (Thursday). After submitting the online application, please print the application form and complete the required approval process, including signatures from your advisor, department/program chair, dean, and other relevant units. Late applications will not be accepted.
