主旨:113學年度第2學期期末成績輸入截止日期及教師申請更改成績截止日期,惠請配合辦理

Subject: Deadline for final grades submission and grade correction applications for the second semester of the 2024 academic year (113-2).

 

說明 Description

  1. 為避免影響學生權益,同時依據本校教師評鑑辦法及專任教師新聘及升等計分標準規定,未於教務處規範時程內繳交學生成績曾提出學生成績更正申請將列入教學行政配合度扣分,請教師留意辦理期程。
    To protect students’ rights and in accordance with the regulations in KMU, failure to submit student grades within the deadline set by the Office of Academic Affairs or submitting grade correction applications will result in a deduction of academic administrative points. Teachers are advised to adhere to all relevant deadlines.
  2. 113-2期末成績輸入截止日期為114年6月27日(五),請各課程主負責教師於期限內至本校資訊系統「T.2.0.03d.課程評分項目成績輸入」輸入期末成績,若需進行總成績微調加分,請填寫「課程大綱、進度表變更申請表」並檢附所有修課學生同意簽名,提出課程大綱評分項目比例變更申請,另設定「平時成績」項目
    The deadline for submitting final grades for the second semester of the 2024 academic year is Friday, June 27, 2025. Teachers responsible for courses are requested to submit final grades via the WAC system (T.2.0.03d. Importation of scores for course grading items) within the specified deadline. If any grade adjustment is needed, please complete the “Course Syllabus and Schedule Adjustment Form” have it signed by all enrolled students, and apply for an adjustment of the grading items in the syllabus to add a new item, such as “Participation Grade,” as needed.
  3. 本處將於114年7月2日(三)以資訊系統輸入之成績為主,陸續寄發學期成績單。
    Semester transcripts will be issued starting Wednesday, July 2, 2025, based primarily on the grades in the WAC system.
  4. 依據學則第30條規定,學生學業成績經授課教師或主負責教師交教務處後不得更改;學生學業成績如有錯誤或有遺漏者,應於成績公布後二星期內,由該教師會同系主任書面申請成績更正,經教務會議查核屬實,方得更正
    According to Article 30 of  Kaohsiung Medical University Academic Regulations, once student grades have been submitted to the Office of Academic Affairs, they may not be altered. If there are errors or omissions, teachers must submit a written grade correction application within two weeks after grades are published, co-signed by the department chair. Any alterations will be confirmed by the Office of Academic Affairs and subsequently reviewed and approved by the Academic Affairs Meeting.
  5. 教師申請更改成績截止日期為114年7月11日,請需更改成績教師至本校資訊系統「T.2.0.18.申請成績異動」填寫資料後列印申請表,並經單位主管用印,送至教務處提教務會議審議;為降低授課教師成績更正率,建議將成績繳交至教務處前,先行公告予學生知悉,並於成績繳交期限進行線上成績修正。
    The deadline for submitting grade correction applications is Friday, July 11, 2025. Teachers requesting corrections must complete the application via the WAC system (T.2.0.18.Application of student's grade correction) to print the form, obtain approval by the head of department, and submit it to the Office of Academic Affairs for Academic Affairs Meeting review. To reduce the need for grade correction applications, it is recommended that teachers announce preliminary grades to students before formal submission and make any necessary online adjustments before the submission deadline.
  6. 教務會議訂於114年7月15日(二)召開。
    The Academic Affairs Meeting is scheduled to be held on Tuesday, July 15, 2025
